To complete an application, you will need to enter your client’s personal details, including (but not limited to):
- Personal details (names, address, dates of birth) of all people living in the household
- Household income, including welfare payments, savings and debts
- Your assessment relating to the child/young person’s situation and the impact it has had on their education and wellbeing
- Information on what items/services you are requesting and a cost estimate
